Change Name and or Surname

Service duration: 20 minutes
Schedule is mandatory: one applicant = one appointment.
The resolution period is approximately 60 days from the date of submission of the request
- preparation of the file with the issuance of the certificate regarding the change of name and/or first name – 20 euros
The citizen of the Republic of Moldova can request the change of name and/or surname based on the following reasons:
- difficult pronunciation, if they are made up of indecent, euphonious, ridiculous expressions, as well as if they have been transformed (schimonosite) by translation;
- the desire to have the name and/or first name corresponding to the nationality of the applicant;
- requesting a simple surname instead of the double name;
- the person’s desire to have the same surname as the spouse’s name or to add the spouse’s surname to their name, if they did not do so at marriage;
- willingness to return to his maiden name if he did not do so upon divorce
- the desire to bear the name of the stepfather (mother) or another person who participated in the applicant’s education;
- justified cases of changing the first name are also recognized, when the applicant actually has a first name different from the one that is registered in the birth certificate in his name;
- other reasons, if they will be recognized as well-founded, a fact that will be described in detail in the respective conclusion by the Civil Status Service.
For the transcription of the civil status document, the applicant will present:
- autobiography;
- explanation, regarding the reasons for changing the name and/or first name;
- birth certificate of the applicant;
- birth certificates of minor children;
- marriage certificate (if applicable);
- divorce certificate (if applicable);
- the applicant’s identity card or passport;
- two 3×4 photos;
- criminal record
- The file for changing the name and/or first name is sent for examination to the Public Services Agency and after approval the Embassy prepares the document and issues the respective certificate.
- The estimated resolution period is 60 days from the date of submission of the request regarding the change of name and/or first name.
- When submitting the application, the fee is paid at the branches of ČSOB banks.
- The change of the surname of both parents entails the change of the surname of the child, and in the case of the change of the surname of one of the parents, the surname of the child can be changed based on the common agreement of the parents. In the absence of such an agreement, the local guardianship authority decides.
- Changing the surname and/or first name of the child who has reached the age of 10 is carried out, in all cases, with his consent.
