Service duration: 20 minutes
The appointment is mandatory: one applicant = one appointment. If a bulletin and passport are requested at the same time, only one appointment will be made, indicating this aspect at the online appointment stage.
Identity card
- within about 45 calendar days: 30 euros
- Within about 21 calendar days: 45 euros
Provisional identity card
- within about 21 calendar days – 26 euros
The identity card is issued in the following cases:
- on first release:
- minors
- persons over the age of 18 who have never had identity documents
- persons who obtained the passport of the citizen of the Republic of Moldova
- citizens of the Republic of Moldova domiciled in the localities on the left side of the Dniester and in the municipality of Bender
- changing the Soviet-type passport (1974 model), provisional identity card or provisional identity card (form no. 9)
- changing, modifying, rectifying or completing personal data
- detecting inaccuracies, non-conformities, identity theft or false identity;
- change of domicile
- in case of change of the name or rank of localities, streets, renumbering of buildings, establishment of localities, of streets, in case of modification, rectification or completion of home address data;
- in case of loss, theft or damage
- expiration of the validity period
- acquiring or reacquiring citizenship of the Republic of Moldova
- requesting the holder to change it
- 2 photos of 10x15cm format + 2 photos of 3×4 cm format, color, matte, on a white background (requirements for photos: the photo will be no more than 6 months old. The photo will represent the image of the subject with shoulders straight, direct gaze, with neutral facial expression and closed mouth. Eyes will be open and not shaded by hair. There will be no head coverings and face coverings, uneven lighting, shadows or reflections on the face or background, red-eye . In case of glasses the eyes will appear without glasses reflections. Dark lenses are not allowed)
- identity card with accompanying form
- valid or expired passport (only in the case of the first documentation of the person with an identity card)
- identity cards with accompanying cards or valid passports of the parents (if a card is requested for a child under 18)
- identity document for internal use, as the case may be, Soviet-type passport (1974 model), in possession to be exchanged
- birth certificate, minor child birth certificate, marriage certificate, divorce certificate (issued in the name of the applicant or the minor applicant’s parent), death certificate of the spouse, certificate of changing the name and/or first name, the notice regarding the cancellation, modification and/or completion of the civil status document. In case of divorce of the child’s parents, the divorce certificate issued in the name of the parent presenting with the child will be presented
- documents required for registration at home or residence or, as the case may be, for modifying, rectifying or completing the home address
- proof of payment of fees (fees will be paid at the consulate with a bank card, at the post office with the receipt received at the consulate or by bank transfer to the consulate’s bank account – details under the heading Consular and related fees).
- The identity card is issued with the accompanying form. The accompanying form is proof of registration at residence and is intended for exercising the right to vote in the Republic of Moldova and does not influence the validity of the identity card.
- The identity card can be issued from birth, it is mandatory from the age of 16 (the citizen of the Republic of Moldova who does not hold an identity card declares that he does not reside in the Republic of Moldova and presents evidence of establishing his domicile on the territory of the Czech Republic, he is considered an emigrant and is entitled to obtain the passport with that mention).
- The identity card is valid from birth to 10 years old / from 10 to 16 years old / from 16 to 25 years old / from 25 to 45 years old / from 45 years old – for life.  ;
- The identity card is only used in the Republic of Moldova, with some exceptions.
- If the person does not have the necessary documents to complete the identity card, a provisional identity card is issued with a validity period of 1 year. A passport cannot be requested on the basis of the provisional identity card, but the document is recognized by the authorities for carrying out other actions in the Republic of Moldova.
- The person who does not have the possibility to be registered at home or residence in the Republic of Moldova obtains the identity card without the given mention.
- The application for the completion of the identity card can be submitted simultaneously with the application for the passport.
- In the case of a minor, the presence of only one parent is requested.
- It is not possible to submit a request for the separate completion of only the accompanying form.
- The applicant’s file is sent to the Public Services Agency, which examines the application and completes the act.
